Obtaining an Immunisation History Statement for your Student

Secondary Schools are required to sight a copy of a student’s Immunisation History Statement (IHS) upon enrolment. The statement must be less than 2 months old.

The easiest way to obtain a statement is by calling the Immunisation History Register (AIR) on 1800 653 809.

If your student is over the age of 14 years, they will have to request the statement. If your child appears on your Medicare Card and is under 14 years old, you may request the statement on their behalf. Please make sure you/your student have your Medicare Card at hand when making the call.

The statement will be posted to the address at which your Medicare Card is registered and will take around 2 weeks.  A copy should then please be given to the School.
